Clinical Care Coordinator – Aged Care

We are seeking an experienced Clinical Care Coordinator to join a well-established aged care provider in the Grafton region.

This is a pivotal leadership role suited to a clinically confident operator who enjoys being the link between strategy and hands-on care delivery. You will play a key role in driving clinical standards, supporting RNs and ensuring compliance excellence within a fully accredited service (2025).

You’ll operate within a structured leadership team:

Facility Manager > 2 x Care Managers > Clinical Care Coordinator > RNs > Regional Support

  • Documentation, ACFI/AN-ACC oversight and compliance

About You
  • Current AHPRA registration (essential)
  • Previous experience in a CCC / Clinical Lead role
  • Strong knowledge of aged care compliance frameworks
  • Confident mentoring and leading RNs
  • Calm, solutions-focused leadership style
